Chronic Disease Management- Everyone has or knows someone with a chronic disease, but most do not know how best to manage their conditions to prevent problems. The health system is built to fix problems not prevent them, I will help you understand the issue and how to prevent problems before they occur
Nurse Advocate- Making sure your voice is heard and rights respected. It also takes someone who knows what usually happens to recognize when its not to say something about it. I am here to help you navigate the health system to access the care you need.
Aging Care Management- Maximize independence and safety to help you live how you want where you want. Proper support can ease the burden of aging and the ongoing challenges that come with it.
Medication Assistance- Getting some help with medications can help prevent your family from worrying... and dangerous mistakes.
Doctor Visit- Accompany you to a doctors appointment to make sure the important questions get asked

I approach patient care with a holistic perspective to promote positive change in the physical, mental and emotional wellbeing of the patient and their family. With a BA in Psychology of Health and Healing and a BSN I use all the skills at my disposal to provide the highest quality nursing care and education to help keep health challenges at bay.
The health system is built around the "sick care" model; fixing problems after they happen with little effort put into preventing problems to begin with. This revolving door might be good for insurance and hospital profits but its not good for the patient. A chronic disease management program helps people be where they want, doing what they want instead of spending time in the hospital leading to functional decline. That's where I come in... with regular visits and communication with your care team and family to get you the care you need before you end up in the hospital.
